Information about online reports’ withdrawal

You can download them by filling in the access data you will find on the proper module. Otherwise, you should withdraw your paper report at the counter.

 

No later than 30 days from the date indicated in the form, patients having the proper system access credentials shall download their own reports on line. After the deadline the report will be deleted. However, on request, a copy of the reports shall be provided at the counters at any time.

How to use the Totem

Poliambulanza’s patients can withdrawal their laboratory examinations’ reports by the Totem at the entrance using a specific bar code written on the receipt issued at the moment of payment together with the date from which they should be available.

You can download them by filling in the access data you will find on the proper module. Otherwise, you should withdrawal your paper report at the counter.

Note about the use of the Totem: The report issued is a certified copy of the original, which instead shall remain deposited in Fondazione Poliambulanza. To ensure the privacy of our patients, each report is unequivocally assigned to login credentials and a specific bar code indicated on patients’ receipt.

The possession of the sheet constitutes therefore delegation to the withdrawal and consultation of the report. The delivery of the printed sheet  through the automatic distribution totem is carried out in favor of the holder of that code and for one time within 30 days following the date of withdrawal indicated.  The access to the report through the website is possible for the holder of the access credentials for 30 days after the date of withdrawal indicated. In the event of any loss or suspicion of possible misuse of the code or credentials, it’s necessary to alert  Poliambulanza Management at Phone No. 030  3515236  to block the report that since that moment  can only be delivered to the person concerned.

Health and Social Services Counter Guide

Fondazione Poliambulanza provides an easy access to Health and Social Services Counter (GASS), a service launched by Lombardy region which enables citizens to have on line access to health and social services. At the same time, it aims at guaranteeing the safety and the full respect of the privacy, since they use their Regional Services Card as a login, a secret password and a disposable code system.

 

PROCEDURE

Today, every citizen can ask for his/her password to be issued at every Public Hospital’s authorized counters by producing his/her Regional Services Card, a valid identity document and giving his/her mobile phone number. Then, patients will be given a form containing the first part of the password, while the second one they will receive later via mobile phone text message together with the instruction for the first log in.

 

FIRST LOG IN

At the first log in, the system will automatically ask you to change the password you were assigned before in two different times.

 

By visiting the Lombardy Region website www.crs.regione.lombardia.it/ssc,  you shall type the last ten figures of the Regional Service Card identification number, enter the password and in real time you will receive on your mobile phone a disposable code in real time, completing the authentication. Consequently, it is possible to have access to your Health Electronic Record and consult on line your medical information and record of examinations and visits. 

 

FURTHER LOG INs

For further accesses you shall type your Regional Service Card identification number, use your customised password and in real time you will receive on your mobile phone a different disposable code every time, so as to complete the application and have free access to your medical reports at every access.

Furthermore, on request, it is available the ‘Reports notification’ service, through which you will be send a text message on your mobile phone when the reports are available on line.

Authorized counters for issuing passwords are numer 9, 10 and 11 and are open from Monday to Friday from 11:00 am to 1:00 pm.
