Care pathways
Fondazione Poliambulanza offers dedicated care pathways to facilitate access to specialist assessments for more complex clinical cases. The service is coordinated by Nurse Case Managers, who serve as the first point of contact for patients and community healthcare professionals.
The Case Manager receives the request, assesses the clinical need, and directly involves the specialist from the most appropriate care pathway, promptly coordinating the initial stages of hospital care, from specialist consultations to the necessary diagnostic tests. This helps reduce waiting times and provides patients with clear guidance from the very beginning.
The service is intended for people with suspicious symptoms or a recent diagnosis who require timely assessment. It is not intended for patients who are already receiving ongoing care within a Fondazione Poliambulanza care pathway.
